The Manheim Public Library is a passport office for the United States Department of State.
Passport services are available Monday through Wednesday from 10 a.m.-7 p.m. Thursday and Friday from 10 a.m.-4 p.m. and Saturday from 9 a.m. – 3 p.m.
Appointments are highly recommended. Call 560-6441 and ask for a Passport Agent. Walk-in service is available but wait time depends on agent availability.
Our passport agents can provide passport photos for $10. This includes 2 identical photos, one of which is required for both new and renewal passport applications.

For most people applying for a passport, you will need the following items:
- Passport Applications – You can obtain a passport application form at the library or download one here: http://travel.state.gov/passport/forms/forms_847.html
- Proof of Citizenship – Birth or naturalization certificate AND a copy of the proof of citizenship
- Photo ID (Valid Driver’s License, previous passport…) AND a copy of the valid photo ID.
- Passport photo (2” X 2” taken within the past 6 months)
Birth Certificate: This must be the original, state issued document with a raised seal, with BOTH parents named. It should say “birth certificate” or “certification of birth” and be issued from the “Department of Vital Statistics.” This original document will be sent with the passport application and returned to you. Go to http://www.cdc.gov/nchs/w2w.htm if you do not have an original or certified, state issued birth certificate, or if BOTH parents are not listed on the certificate.
Naturalization Certificate: This must be the original or a certified document with a raised seal. This will be sent with the passport application and returned to you.
Valid Photo Identification with Signature: Your driver’s license is the best choice. If an out-of-PA license is presented, an additional photo ID must be shown.
Passport Photos: 1 photo with strict specifications must be submitted. You can have a set of 2 taken at the library for $10 per person.
Social Security Number: To avoid delay in processing, applicants are advised to include their social security number.
NOTE: Children 15 and under need BOTH parents to be present, OR provide a notarized Consent Form DS-3053. Teens ages 16 and 17 must have proof of parental consent or a parent present at the time of application.

The U.S. Department of State accepts only personal checks or money orders for each passport application, and the library accepts money orders, checks or cash for their execution and photo fees.
If possible, please complete the proper application form BEFORE your appointment time. If not, we are happy to help.
No white out is allowed to be used on a passport application. If a mistake is made on the form, just cross out the error with one line and write the correct information above or beside the error.

Manheim Township returned check fee is $30.

Same fees as above to the DOS; however, since applicants complete and mail the form, check and previous passport themselves, there is no library fee. We can take the required photo for $10.
