Manheim Township Public Library is a Passport Acceptance Facility for the U.S. Department of State.

Please note: U. S. Department of State fees must be paid with a CHECK or MONEY ORDER.

MTPL Acceptance Facility fees can be paid with CHECK, MONEY ORDER, or CASH.

CREDIT AND DEBIT CARDS ARE NOT ACCEPTED.

 

Passport Basics

New Passport Applicants

You will need to bring the following items:

  1. Completed DS-11 Passport Application
  2. Proof of Citizenship – Birth Certificate, Naturalization Certificate, OR expired (but not damaged) passport
  3. Photo ID – Valid Driver’s License or previous passport (see the DOS website for other acceptable forms of identification)
  4. Photocopies of the Proof of Citizenship and Photo ID
  5. Passport Photo – Taken at the appointment time or brought from another photo site (see the DOS website for the detailed photo requirements)
  6. Payment – For U.S. Department of State fees one check or money order for each application (made out to U.S. Department of State), and for Acceptance Facility fees a separate payment in cash, check or money order (made out to MTPL). See more information in the Fees chart below.

(Failure to bring ALL items will result in rescheduling/cancellation of appointment.)

NOTE:

For children 15 and under, BOTH parents must be present, OR provide a notarized Consent Form DS-3053.

Teens ages 16 and 17 must have proof of parental awareness of  intent to apply. Parental awareness can be shown either by supplying a notarized statement of consent from at least one parent or guardian, or by bringing one parent or guardian to co-sign the form. Renewal Applicants

Most adult passports that have not been expired for more than 5 years are eligible for a renewal by mail. These renewal applications are submitted by the applicant and are not processed by an acceptance facility. Renewal forms (DS-82) are available at our facility, and passport photos may be taken on site. See the DOS website for more information about renewals.

 

Passport Application: First time applicants will use the DS-11 form regardless of their age. Those who are renewing an adult passport that was issued in the last 15 years can use the DS-82 form and are eligible to submit their renewal by mail. See the DOS website for more information on which form to use.

Please complete the application BEFORE your appointment. (Do not use correction tape/liquid. Cross out mistakes with one line and write in correction.)  

Birth Certificate: Original or certified state issued document with at least one parent’s name on the document. It should say “birth certificate” or “certification of birth”.   The original document will be sent with the passport application and returned directly to you.

Naturalization Certificate: Original or a certified document with a raised seal. This will be sent with the passport application and returned directly to you.

Valid Photo Identification with Signature: Driver’s License or State ID. If an out-of-PA license is presented, additional photo ID must be shown.

Passport Photos: 1 photo with strict specifications must be submitted. These photos can be taken at another provider and brought with you at the application time, or a set can be taken at the library for $12 per person.

Social Security Number: Applicants are advised to include their social security number to avoid processing delays.

 

Service Times

Routine Service: Average wait for a passport is 6-8 weeks.

Expedited Service: Receive your passport in about 2-3 weeks.
